Semen is collected on the day of the ovum pick up and
the male partner must contact the reproductive
biologist to determine what time they are required to
give the sample .The semen is generally processed
using the double wah and swim up method.Depending on
the semen report semen can be used for IVF or ICSI.
ICSI – Intra Cytoplasmic Sperm Injection :
The direct injection of sperms into the eggs obtained
after ovum pick up frequently permits the
establishment if pregnancy in even the most difficult
types of male infertility including men who have no
sperms in the semen sample because of vasectomy,
blocked ducts failure to ejaculate because of spinal
cord injury , non obstructive azospermia and
conjential absence of vas a non surgical sperm
aspiration is performed .TESA /PESA is an easy and
quick method to obtain adequate number of sperms for
ICSI. TESA .PESA is performed under sedation and is
painless and rapid .a tiny needle is used to extract
sperms directly from the testis. Aspiration of 100/
200 sperms by is enough to achieve pregnancy when
combined with ICSI .
IVF:
In a natural cycle fertilization occurs in the outer
part of the fallopian tube. In a IVF cycle the
harvested eggs are mixed with sperms ( 1 lac
sperms/egg ) in a Petri dish. Fertilization is usually
checked after 16-18hrs. IVF is generally done for
patients in whom the semwn parameters are normal.
CAUSES OF CANCELLATION OF TREATMENT CYCLE PRIOR TO
OPU:
The aim intensive monitoring programme is to obtain
healthy mature eggs ,Thus if the cycle is
unsatisfactory. It may be cancelled at any stage.
1) The follicles are developing in a necessary ovary .
2) When very few follicles are developing which would
decrease the chance of obtaining at least one mature
egg.
3)There are no follicles developing at all. This means
higher doses must be administrated next time.
4) The blood oestrogen levels are abnormal ,they may
be too low or too high for the development of healthy
eggs or according to the patterns seen over the last 5
years unlikely to lead to pregnancy
5) The follicles may have ovulated prior to ovum pick
up .Occasionally some women ovulate earlier than
expected or ovulate without our being able to detect
the time when ovulation commenced.
6) Sometimes ovarian cysts develop in response to the
drugs .These are neither serious nor harmful but the
cycle may have to be abandoned as it is difficult to
satisfactory monitor the growth of other follicles or
the cyst may have to be punctured .they usually
resolve within one month .
7) Rarely . due to circumstances beyond our control,
such as equipment overload , equipment failure or
patient overload , you may be advised that your
current treatment cycle is to be abandoned.
